The application

The “forMyHeart” application is addressed to individuals suffering from arterial hypertension/CVD, as well as to treating physicians and nurses. The aim of the application is to provide support for comprehensive and personalized self-management of the disease.

The application assists users with hypertension in the self-management of their condition and supports their decision-making process through the collection, recording, and monitoring of their health data over time. It serves as a tool for recording and analyzing user data and measurements, as well as for generating outcomes that reflect daily progress in relation to specific goals set by the user.
The application supports the patient-user through motivational and/or warning messages, depending on their measurements, and also sends an SOS notification to predefined contacts (relatives, friends) in the event of a potential risk.

Additionally, the patient-user may, if they wish, grant access to their treating physician so that the physician can monitor the data and measurements recorded in the application. In this way, the physician can provide immediate care and support to the patient, while simultaneously reducing costs, time, and the need for travel between them.

The objectives

The objectives of the application are the utilization of innovative approaches and cutting-edge technologies for:
  • Easy recording, monitoring, and management of data that affect the disease
  • Detection and management of atrial fibrillation episodes
  • Support for CVD patients in decision-making, medication adherence, and education/information about the disease
  • Strengthening patient–physician communication
  • Immediate physician feedback in cases requiring attention
  • Comprehensive remote monitoring of patients by physicians
  • Optimization of delivered healthcare services
  • Support for the patient’s family and close social environment
  • Analysis and utilization of medical data by academic/clinical institutions
  • Integration with third-party apps/devices for more complete management of the disease and comorbidities (e.g., diabetes)
  • Enhancement of research in the field of cardiovascular diseases through digital applications (mHealth)
  • Networking between medical-sector organizations, enabling coordination of joint actions for mutual benefit

Patient

Patients with hypertension

Young & middle-aged people, AD stage 1, 2, 3 etc., familiar with the new technologies, interested in their physical condition, diet (salt intake, alcohol, smoking), engage in daily activities, have a social life and wish to adhere to their systematic treatment.
Research

Research organizations

The data collected will be utilized by research organizations (research institutes, university laboratories, providers of health services and dissemination of knowledge, medical associations) to exploit the data that will be gathered.

Sufferers' relative/friendly environment

Ability to monitor patient progress.
Doctor

Doctors dealing with cardiovascular diseases

For more comprehensive patient monitoring, pattern recognition, immediate feedback and optimization of provided services.
